從臨床檢體分離出Kodamaea ohmeri之正確性

Kodamaea ohmeri isolation from a wound pus culture, is it true?

Objective:
Isolation of Kodamaea ohmeri, teleomorph of Candida guilliermondii, from clinical samples is very rare. Most case reports in the literatures were cancer or immunosuppression patients. In May of 2019, first Kodamaea ohmeri strain was identified by VITEK® 2 system from a wound pus of a nasopharyngeal lymphoma patient in Taichung Tzuchi Hospital. Due to their rarity in nature and treatment considerations, we use another identification methods to confirm the species of this isolate.
Materials and Methods:
The culture mediums used during this pus culture were blood agar plate, eosin methylene blue, chocolate agar, sabouraud dextrose agar and thioglycollate medium. The identification methods were YST ID card of VITEK® 2, BD™ Bruker MALDI Biotyper™ and internal transcribed spacers (ITS1-ITS2) sequencing.
Results:
The identification result was Kodamaea ohmeri by YST ID card of VITEK® 2 system with 99% confidence. The result of BD™ Bruker MALDI Biotyper™ was Kodamaea ohmeri with a rating of 2.14. The internal transcribed spacers (ITS1-ITS2) sequencing result was also Kodamaea ohmeri compared to NCBI data base. The isolate was sensitive to Fluconazole (MIC=8 ug/ml), Caspofungin (MIC≦0.25 ug/ml) and Voriconazole (MIC≦0.12 ug/ml).
Conclusions:
Kodamaea ohmeri was rare in clinical isolates. Further confirm of identification result by at least another one method was obligated for the microbiology laboratory. Via this case, we developed an active identification process for questionable clinical isolates to prevent error reports. This was an inevitable responsibility under the consideration of patient safety.
